Coaches,
Click on the links below and print off the form for your Athletes. Have every person fill it out. You only need to do this once a year for eligibility in all the sports. Just check the sports you will be participating in and get the proper signatures. Please encourage friends of other faiths to join you on your teams. This is a great way to get to know them better!
